Intake, Assessment and Orientation

INTAKE

The Intake Form should capture relevant information on the student's employment status, childcare issues, motivation, goals, career interests, academic strengths and weakness (self-assessment), academic experiences, learning styles, and academic assessment information. It will also help to determine pertinent information for the college application process. For example, you will know in advance what scholarships a student can apply for or if a student is not eligible for financial aid.

For the ABE-to-College Transition Project, we asked our participating programs to track student attendance throughout college. Tracking students' attendance in college allowed programs to track their students' successes. Should you want to do this too, you must obtain written permission from your students to allow your program to track their attendance. Without written consent, the college registrar will not give you attendance information. You will need this information as your students transition. After the intake form is completed, the counselor and potential students should discuss their long-term and short-term goals and their ability to make a commitment to the program.
